Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions about physical occupational therapy coverage:
Do I have physical therapy insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per physical therapy session?
Is approval required from my primary care physician?
Payment
Cash, check and all major credit cards accepted for payment.
Copays - all copays are due at time of service
Cancellation Policy
We respectfully request 24 hours for cancellation. We do understand that situations arise. However, if you do not show up for your scheduled appointment , please know that there will be a $25 fee applied to your account.
